Several days ago, an acquaintance of mine had to go to the ER. To rule out the possibility of a larger problem, the doctor suggested they get a CAT scan. Right away, even before the procedure, registration had come back with the bill, which was a large mortgage payment for 95% of the country. Saying that one’s head is in the clouds suggests that he or she is isolated and detached from the realities on the ground below them. I suppose it could be interpreted as taking a high altitude view of something, but that wasn’t the impression I got when my teachers (and later, bosses) encouraged me to come back down to Earth.
